Altus Dental Plans

Plans are typically organized by coverage: Preventive, Minor and Major Restorative and Orthodontic. Employers can select benefit levels, co-insurance amounts, deductibles and annual maximums for all their coverage options. Take a look:

Preventive/Diagnostic Services - You choose co-insurance options: 100% or 80%.
Coverage includes: Two oral exams annually, two cleanings per calendar year, annual bitewing x-rays, full mouth x- rays every five years, two fluoride treatments per year for children.

Basic/Minor Restorative Services - You choose co-insurance options: 80% or 50%.
Coverage includes: Fillings, root canals for permanent teeth, oral surgery not covered by your health plan, bridge & denture repair, sealants for children under 16, periodontal maintenance.

Major Restorative - You choose co-insurance options: 80%, 50%, 0%.
Coverage includes: crowns, dentures and bridges.

Basic/Minor and Major Restorative Deductible Options - You choose: $25, $50, $100. Per member, per calendar year. Family maximums of two or three times per individual available.

Calendar Year Maximum Options - You choose: $500, $750, $1000, $1200, $1500.

Additional Coverage Orthodontic - You choose co-insurance options: 50%, 0%.
Separate lifetime maximum, you choose from $500, $750, $1000, $1200 or $1500 per child. Coverage includes braces and related services for dependent children under age 19.

Student Rider - from age 19 up to age 26 while attending school full time.
