National Provider Identifier (NPI)

Compliance Deadline Fast Approaching

May 2007 is the deadline to begin using your NPI – a unique, new health care provider identifier for electronic transactions, including claim submissions. Please continue to include your state license number and tax payer indentification number on your dental claims.

Please note: You must have an NPI to use our E-Office services, including BenefitCheck^SM and Claims Exchange.

If you have not obtained your NPI, please go to the government website: nppes.cms.hhs.gov or call the toll free number: 1-800-465-3203.

Once you receive your NPI, please send or email a copy of your notice from Fox Systems, Inc. to:
