Commonly Asked Questions
Q. Does Altus Dental sell individual policies?
A. Altus Dental sells group programs to Massachusetts-based businesses. Altus Dental does not offer individual memberships through direct pay programs. Members of AAA Southern New England who live in Massachusetts can purchase individual dental coverage with us through AAA. Click here for details.
Q. Do I have to go to a particular dentist?
A. You always have the freedom to choose any dentist, however your out-of-pocket costs will probably be higher at a non-participating dentist. As a member of Altus Dental, you enjoy distinct advantages when you visit a participating dentist. That's because participating dentists agree to accept the allowance as full payment. That means that they will not bill you for any difference between the amount we allow and their actual charge.
Q. Can my dentist join the network?
A. If you'd like us to invite your dentist to join the network, click here to send us your dentist's name and address. An Altus Dental Representative will be sure to contact you and your dentist. If he or she decides to participate and meets Altus Dental's credentialling and geographical selection criteria, we'll enroll him or her in the network.
Q. What does my plan cover?
A. One of the benefits of Altus Dental is our total flexibility in plan design, so there is no one answer to this question. You can check your specific benefits coverage information now with BenefitCheck, call InfoLine - our automated phone system - or check the benefits literature available through your company.
Q. How do I check to see if I've met my deductible?
A. Visit BenefitCheck, our online benefits and eligibility feature, or call the InfoLine, at 1-877-223-0588, enter your ID number and follow the prompts. InfoLine is an automated system that allows you to check on your specific benefits.
Q. Do I need a referral to have a procedure done by a specialist?
A. You do not need a referral but we strongly encourage you to use a participating dentist to maximize your benefits. You may also want to have the specialist submit a pre-treatment estimate to determine any potential out-of-pocket costs.
Exams and consultations by specialists may not be covered by your plan so check your specific benefits by visiting BenefitCheck.
Q. Do I need to submit a claim form?
A. Participating dentists will take care of filing claims for you. Some non-participating dentists may submit them on your behalf. Call the dentist's office before you visit to make sure, and if you need one, you may download one here.
Q. If I have to send a claim form, where do I send it?
A. You may send your completed and signed claim form to:
Altus Dental Insurance Co., Inc.
PO Box 1557
Providence, RI 02901-1557
Q. What do I do if I've lost my ID card?
A. Email us, or call Customer Service (1-877-223-0588) and we will mail a replacement card to you within two weeks.
Q. What if I have a dependent/student living out of state?
A. You may have access to participating dentists through the national CONNECTION Dental network. It's one of the largest dental networks in the nation, with more than 51,000 locations - including general dentists and specialists - nationwide. |
