Q.  What is Altus Dental for 1?
A.  It’s an innovative way to get individual dental insurance coverage in Massachusetts from Altus Dental Insurance Company.

Q. Who can enroll?
A. You are eligible to enroll in the Plan if you are a Massachusetts resident. In the case of a child who is less than 18 years of age, a parent or legal guardian must contract on behalf of the dependent child for the benefits described in this Certificate of Coverage. The parent or legal guardian is responsible for the dependent child’s compliance with any and all terms and conditions outlined in the policy. This plan covers only the individual enrolled.

Q. How much is the enrollment fee?
A.  There is a one-time non-refundable enrollment fee of $35 for your new coverage. This fee will be charged to your bank account or credit/debit card at the time of enrollment.

Q. How do monthly payments work?
A. We will deduct your monthly payments directly from your bank account or credit card (VISA, MasterCard or Discover). If you choose to pay via direct withdrawal from your bank account, you will receive a $2 per month discount.

Q. How do I know which dentists are available for my plan?
A. Click here to see a list of dentists available to you. You can search based on location, specialty or name.

Q. Do I have to go to a particular dentist?
A. You always have the freedom to choose any dentist; however, your out-of-pocket costs may be higher at a non-participating dentist. Participating dentists agree to accept our payment as payment in full, minus any applicable deductibles or co-insurances. You are protected against being billed for any remaining balance. A participating dentist will also file your claims directly with us and handle all paperwork.

Q. What is a waiting period?
A.  A waiting period is the amount of time you must wait from the date your coverage begins (also called the effective date) before a specific procedure is covered. If your plan has a waiting period, it will be noted in the Benefits Summary and the Schedule of Covered Dental Procedures.

Q. What does "out of network" mean? How does it impact my costs?
A. "Out of network” means that you have chosen to receive services from a dentist who does not belong to our contracted dental network. Your out-of-pocket costs may be higher because we do not have a contractual agreement with the dentist. You must first pay $250 for any of the services listed in the “Schedule” before we pay for any covered service. This upfront payment is called a “deductible.” Once the deductible is met, you are responsible for the “You Pay” amount for that particular covered service, plus any difference between what your dentist charges and the amount we pay. Our payment is the lower of the amount the dentist actually charges, or the amount we determine to be what most dentists in the same geographic area will charge for that service.

Q. How do I cancel my coverage?
A. You may cancel your coverage by logging in to your account on our website and selecting the cancellation action. You must notify us at least 30 days in advance. Cancellation is effective as of the last day of the month for which the premium has been paid.

The following are some dental terms that may be helpful to know:

Allowance: The amount we base reimbursement on for a covered service or procedure.

Co-payment: The amount you pay for covered services.

Covered Services: Those services and procedures listed in the Benefits Summary and Schedule of Covered Dental Procedures.

Effective Date: The date, as shown in our records, on which your coverage begins.

Endodontics: A specialty of dentistry that deals with the treatment of dental pulp diseases (nerves, blood vessels and other tissues within the tooth). A root canal is an example of endodontic treatment.

Non-participating Dentist: A dentist who does not have a contract with Altus Dental Insurance Company, Inc.

Participating Dentist: A dentist who has a contract with Altus Dental Insurance Company, Inc.

Pedodontics: A specialty of dentistry concerned with the treatment of children.

Periodontics: A specialty of dentistry concerned with diseases of the gums and other supportive structures of the teeth.

Prosthodontics: A specialty of dentistry concerned with the replacement of missing teeth by bridges and dentures.
